Registration
If you live within our practice boundary, please ask the receptionist for a registration form for each person wishing to register at your address. The form will ask you for personal details such as your name, address, postcode, telephone number, and also for details of your previous doctor and your previous address. Without these details we cannot process your application. Once you are registered, your medical records will be forwarded to us from your last GP practice.
If you are coming to live here from abroad we will need to know your date of arrival in this country (and your date of leaving the U.K. if you have lived here in the past).
If you have just left the Armed Forces, we will need to know your enlistment date and leaving date, and your address before enlisting. We also need your service number.
All new patients will be asked to complete a New Patient Questionnaire prior to registering. All patients aged 5 and over will be offered a New Patient Health Check.

Making an Appointment
You may telephone the surgery anytime during our opening hours 8.30-6.30 pm. Unless you need to be seen on the same day, it is advisable to avoid our peak call time 8.30-9.30 am. You may book an appointment up to 5 weeks in advance with the doctor of your choice, or you may ring first thing in the morning if you have an urgent medical problem (but this will not necessarily be with the doctor of your choice). Alternatively you can book an appointment online (please ask Reception for details)
All our nurse run clinics are by appointment only. Please phone the number below as soon as you know that you need to be seen by one of our nurses.

Telephone Advice
There are two main ways for patients to seek telephone advice:
- The Receptionist sends an email to the relevant Doctor or Nurse with details of the patient's query. The Doctor or Nurse responds by email to Reception. The Patient rings back for the Doctor's or Nurse's response.
- You may book a telephone consultation at a specific time with the Doctor or Diabetes Specialist Nurse.
The Health Visitors are available for telephone advice on minor ailments in babies and young children, Mon-Fri between the hours of 9-10am and 4-5pm, on 01625 441996.

Home Visits
These are for patients who are genuinely unable, through illness or infirmity, to come to the surgery. Please phone the surgery before 10.30 am if possible, so the doctors can plan their visits. The receptionist will ask for details of symptoms so that the doctor can decide how urgently a visit is needed. One of the doctors may phone you before coming to see you to determine if a home visit is necessary. You will not necessarily be visited by your Doctor of choice.
Please remember that a doctor can see 3 or 4 patients in surgery in the time it takes to make one home visit.

Repeat Prescriptions
Your Doctor will tell you if you may have an item on repeat prescription.
You may request your repeat items online (please ask for details at Reception).
Alternatively, you may bring the repeat side of your prescription (the white, tear-off side) down to the surgery (having ticked the relevant boxes) and place it in the repeat prescription box in the entrance hall, or you may post it to us with a stamped addressed envelope. We regret we are unable to take prescription requests over the phone.
Please allow 2 working days for the processing of repeat prescriptions, and longer if a doctor is reviewing your medication.
We have an arrangement with certain local pharmacies whereby we can send your prescription straight to them, and you can then collect your medication ready-dispensed. Please ask your pharmacist for details.

Test Results
The doctor or nurse will, at the time of arranging any test, discuss with you how you will get to know the result and how soon it will be available. If you are phoning Reception about results, please call between 2-4pm.
The receptionist will either read out the doctor's comment or will send a message to the doctor or nurse who requested the test. This will probably mean you will be asked to phone again for the doctor’s or nurse’s comments, or may sometimes be asked to make an appointment.

Completion of Forms & Charges for Services
The doctors are often requested to provide certain services which fall outside NHS provision. For these services the patient is liable to a charge in line with the British Medical Association recommendations. Some examples are given below:

Ambulance Patient Transport Service (PTS)
Non-emergency ambulance transport is a service provided by your local NHS ambulance provider.
This provides access to transport for patients with medical and/or mobility conditions that prevents them from using other forms of transport. Department of Health guidance states that a clinical need for treatment does not imply a medical need for transport.

Travel Vaccinations
If you are planning to go abroad and you believe that you may need vaccinations, please come into the surgery and fill in a travel form at least 8 weeks before you travel. After completing the form you will be asked to phone the surgery to speak to the Travel Clinic nurse to plan your vaccination schedule and appointment dates. She will also advise you of the charges for travel vaccinations.
