How to Register
If you live within our practice boundary you will need to print off and complete a Registration Form and an Adult New Patient Questionnaire or Child New Patient Questionnaire for each person wishing to register at your address.
The form will ask you for personal details such as your name, address, telephone number, details of your previous Doctor and your previous address. For patients who have come from overseas we will require your immunisation records (in English).
Once your are registered, your medical records will be forwarded to us from your last GP practice.
Out of area registration will not be accepted or processed as we do not accept patient outside our practice area.

Register with the Practice
To join the Practice you must live within our practice boundary (if you are unsure, please ring Reception on 01625 529421 to check).
- To register you need to complete a Registration Form (GMS1) and a New Patient Questionnaire (see below for links to these forms) for each member of the household. These forms are also available at Reception if you prefer to collect a paper copy.
- Once all forms have been completed, please bring them to Reception with photo ID and proof of address.
