Change of Address

You can now update your address via accuRx. You can state on the enquiry form what your name, date of birth and current address is, and then confirm the new address and date of change. Alternatively, come into the Practice to pick up a form or download one here. Bring this to the practice with your ID and we can facilitate the address change.

If you made the address change via accuRx, we will try to contact you by phone for verbal confirmation that you made this request. If we are unsuccessful in contacting you, we will send you a text. You only need reply if you did not make the request.

If you are requesting the change of address for children at the address for whom you have parental responsibility, we can make the amendment for your children. However, if there are other adults in the house, please be advised that we will attempt to contact them by phone and text for confirmation of the address change.

All of the above is subject to you residing in our boundary area. If you have moved further afield, you will need to register with a GP closer to home as we no longer allow Out of Area Registrations.

Change Of Name

To advise the Practice of a Change in Name, you will need to come into the Practice to complete a form and also provide photographic and documentary evidence. E.g. if you have changed your name due to marriage, you will need to bring in your birth certificate and also marriage certificate. If you have a printer at home you can download and complete the Change of Name Form, but would still need to come to the Practice to have your identity verified.
